Collaboration in Action: How HIVE and PA Navigate Are Strengthening Schuylkill County
When organizations come together with a shared purpose, communities feel the difference. In Schuylkill County, that spirit of collaboration is alive through the HIVE initiative and a growing network of partners using PA Navigate to better support their neighbors. From learning together to strengthening referral connections, local agencies are finding new ways to ensure no one falls through the cracks. This story is a reminder that when people, trust, and the right tools come together, real change happens.
From Conversation to Connection: Improving Behavioral Health Access in Fulton County
In Fulton County, local partners are strengthening access to behavioral health services by turning community voices into real connections. Through collaboration between Fulton County Medical Center and the Fulton County Family Partnership, healthcare and social service providers are using PA Navigate to more easily connect individuals and families to the behavioral health resources they need. By increasing awareness and adoption of this shared referral platform, agencies are improving coordination, reducing barriers to care, and ensuring that community members can find support faster when it matters most.
Finding the “Yes” and Closing the Loop on Food Access
PA Navigate is helping Meals on Wheels of the Greater Lehigh Valley say “yes” more often by connecting older adults and homebound residents to food assistance and critical community supports faster and more efficiently. By enabling real-time referrals from healthcare providers and community partners, PA Navigate reduces delays, improves coordination, and ensures no request for help ends in a dead end. When Meals on Wheels cannot meet a specific need, the platform helps staff quickly connect individuals to other trusted organizations—closing the loop and strengthening the care continuum. Since adopting PA Navigate, Meals on Wheels has dramatically improved intake-to-delivery time, expanded access for residents facing food insecurity and social isolation, and successfully followed through on 96% of referrals—ensuring whole-person care reaches people where they live.
