Pennsylvania Social Care Summit: Highlights from the Keystone State
CAAP recently joined partners from across Pennsylvania at the Findhelp Pennsylvania Social Care Summit in Philadelphia, where leaders from healthcare, community-based organizations, government agencies, and technology partners came together to discuss the future of connected care.
During the event, Kat Royer, Karen Stone, and Beck Moore shared how PA Navigate is helping build a more connected system of support across the Commonwealth.
They appreciated the way the Summit highlighted several things:
PA Navigate is creating a "no wrong door" approach to care by connecting healthcare organizations, community-based organizations, health information organizations, and payers.
The platform helps community-based organizations increase visibility, improve efficiency, and access technology and data that can strengthen their work.
CAAP plays an important role by providing training, support, and trusted guidance to organizations using PA Navigate.
A statewide system like PA Navigate helps reduce barriers and makes it easier for individuals and families to find and access services with dignity.
The Summit continued to emphasize the importance of collaboration, innovation, and strong partnerships in building a more connected social care system for all Pennsylvanians.
