Fayette County Community Action Agency, Inc. - PA Navigate Testimonial
Fayette County Community Action Agency, Inc. (FCCAA) is committed to the full evolution of PA Navigate throughout its service area. Alongside the Community Action Association of PA, FCCAA plans to partner with fellow nonprofits, healthcare providers and insurance payers to begin to coordinate “whole-person care” through PA Navigate.
As a result of these developing partnerships, all parties will have a way to communicate seamlessly and to support residents in-need. This will ensure that while healthcare providers and insurance payers address the physical and behavioral health needs of residents, nonprofit organizations are addressing patient Social Determinants of Health.
FCCAA has incorporated all agency programs into the PA Navigate online platform with the support of CAAP. As FCCAA has gained familiarity with the platform, key staff realize the immense opportunity the platform has; its full integration by Fayette County nonprofit organizations, behavioral healthcare and physical healthcare providers will become a catalyst for change for Fayette County. Not only can project partners utilize PA Navigate to communicate but residents can directly access the platform to learn about services and contact nonprofit organizations directly.
“As this coordination evolves, FCCAA will be positioned to work with partners to ensure residents can better address their socioeconomic, behavioral health and physical needs, collectively,” said FCCAA’s CEO Kim Hawk. “We see PA Navigate as essential component of our service delivery system.”
Since the integration of PA Navigate at FCCAA, dozens of Fayette County residents have been able to address their food insecurities by accessing one of the FCCAA Food Bank’s 56 volunteer food pantries. The seamless self-referral or partner referrals the Food Bank has received through PA Navigate is helping the agency to ensure more residents are able to access perishable and nonperishable foods close to their homes.
