The PA Navigate Trusted Network

In April 2024, the Community Action Association of Pennsylvania (CAAP) joined the PA Navigate project team.  CAAP is responsible for bringing the Community Based Organization’s (CBO) perspectives to the project as well as for training and assisting Community Based Organizations that wish to join the PA Navigate Trusted Network. 

Part of an integration project with a provider or payer includes having the provider or payer identify the most trusted or relied upon Community Based Organizations in their community.  Findhelp ensures that those Community Based Organizations are listed on the PA Navigate platform and then CAAP’s team of Community Engagement Managers (CEM) reach out to the Community Based Organizations and invite them to trainings or assist them with claiming their listings and updating their program cards. 

As of March 31, 2025, our integrated providers and payers have recommended 1329 programs.  Of these, the CAAP CEM team has successfully connected with nearly 600 programs.  Of these nearly 600 connections, 244 programs have already done the training and other work required to join the PA Navigate Trusted Network, earning the below distinction on their program card and optimizing their programs in search results. 

We are excited to report that the closed loop rate for the PA Navigate Trusted Network as of March 31, 2025, was 53%! 

Benefits of Joining the PA Navigate Trusted Network: 

·       Higher Visibility: Trusted Network programs are prioritized in search results. 

·       Preferred Referrals: Clinical users are encouraged to send referrals to Trusted Network organizations first, leading to greater service connection opportunities. 

·       Data and Reporting Support: Members gain access to improved reporting on referrals and outcomes to help demonstrate impact. 

·       Funding Opportunities: Some grant and incentive programs may prioritize Trusted Network members, particularly during pilot phases. 

·       Credibility and Trust: Being part of the Trusted Network signals to other organizations and users that your agency is a reliable, responsive partner.

·      Step 2: Create an account for your organization in PA Navigate. 

·       Step 3: Agree to certain service expectations, including timely referral responses and updating referral statuses when services are completed.
