  • A Trusted Network organization to highlight is Outreach Center for Community Resources which is also participating in the phase 2 incentive program. This organization is a designated Pennsylvania Family Center in Scranton, PA that provides evidence-based programs tied to prevention, education, and intervention to individuals and families. Outreach Center for Community Resources has become a trusted, responsive partner with utilization of PA Navigate; achieving a 100% acknowledgement and closed lop rate. This means that every referral is seen, responded to, and followed through on, reflecting a strong commitment to supporting the local community and improving outcomes for the individuals they serve.

  • A new, trusted network partner to also highlight is Freedom Transit-Washington County Transit Authority as they are excited to utilize PA Navigate to showcase their programs and services in a user-friendly way.

“As a community resource provider, we understand transportation is an essential service that can limit community members if they are unaware, they may be eligible for service/s. When transportation is available, it can open the world to improved health outcomes, community interconnectedness, essential shopping, education, and the entertainment that enriches our lives. With PA Navigate, we can present all our services in a user-friendly format. We are thrilled that community members will be able to find us more easily and that we can now follow up with them efficiently through the referral program.”
